REMINDER: November 2009 VBS commitments due in less than 2 weeks!
REQUIRED INFORMATION DUE NO LATER THAN September 10, 2009:
1. Number on team, including number of males, females, couples
2. First and last names in rooming list of 2 per room. If parent/child sharing room, please indicate so we can make sure room has 2 beds. If you have odd number of people coming to serve in lake area churches, someone will have to either be in single room or share with someone from another team. Please indicate if person would prefer to stay in single room or share and we will honor that request. Also will put teams in rooms side-by-side as much as possible. Unfortunately the hotels do not have rooms with more than 2 (twin-sized) beds.
3. Flight information: airline + date, flight number & time of arrival + date, flight number & time of departure.
4. Number of meals team will need for day of arrival. If team arrives after 6 p.m., snack can be offered as substitute for full meal at lesser price than full meal. Teams arriving before 1 p.m. will probably want 2 meals. Teams arriving after 1 p.m. may prefer to just have evening meal instead of 2 meals. Evening meal is usually served around 6 p.m.
